Application Requirements for a Corporate Account:
Verification of Bona Fide Business Entity (One of the following)
- Business license
Articles of Incorporation/Organization - Corporate verification with State or Federal Government Sales tax identification
- Sales tax records
- Secretary of State trade name registration (accepted on sole proprietor only).
Verification of Business Address (One of the following)
- Lease (last page with landlord and lessee signatures)
- Utility bill in company name
- Invoice from vendor (i.e., Title Company)
- Voided business check
Additional Requirements
- Personal or business credit report pulled by Premium Credit Bureau
- Copy of account holder's drivers license
- On Site inspection by 3rd party paid by the applicant ($75.00)
- Membership application (Online Submittal)
- Specific industry license based on state requirements (i.e. mortgage license)

Application Requirements for a Branch Account:
- Corporate Letter of Authorization
Membership application (Online Submittal) - Personal or business credit report pulled by Premium Credit Bureau
- Specific industry license based on state requirements (i.e. mortgage license)
- Copy of account holder's driver's license
